Description Job Opportunity: Registered Nurse - Nurse Navigator Facility: Sutter Health Eden Medical Center Location: Castro Valley, CA Employment Type: Travel/Contract Shift: Day (5x8 Hours) 8:00 16:30 Start Date: 3/24/2025 Job Duration: 13 weeks Compensation: Rate Type: Hourly Over Time: 60 total hours in 1 week: 1.25% Double Time: None On-Call: 7$ Call Back: 1.35% Holiday Pay: 1.25% Additional Information: Nurse (Stroke) Navigator License/Certifications are: CA RN BLS-Basic Life Support. Possible weekends with overtime 5 years recent relevant acute care experience for stroke patients or as a nurse navigator assisting patients with post discharge babrriers. Comfortability with chart review to identify gaps in stroke care and ability to close the loop with the primary nurse in a collaborative manner.

Effective communicator related to stroke education to both staff and patients. Partners with the stroke coordinator to oversee the execution of procedures and adherence to proper stroke protocols and develops and/or administers performance improvement initiatives for the stroke program. Assumes responsibility for program quality and compliance, and the education and competency of nursing staff.

Serves as a clinical and educational consultant to the nursing staff, medical staff, management, patients and other members of the health care team regarding the program. In conjunction with nursing staff, medical staff, and management, establishes a clinical program education program for orientation and continuing education. Contributes to community awareness through outreach program involvement and or development.

SKILLS AND KNOWLEDGE: Knowledge of medical terminology required. Ability to ensure follow through with responsibilities and communicate with a varied level of health care staff. Knowledge of third-party reimbursement helpful.

Verbal and written communication skills. Microsoft Office software experience required, including Excel and Word
