Specialist Occupational Therapist - Home First Closing date: 21 June 2026. Opportunity to join the Urgent Community Response Team, based with the Community Integrated Response Team. The role will work across Frailty Virtual Wards, Urgent Community Response and Home First.

The service operates extended hours 7 days per week providing cover 8 am–8 pm. You will be expected to cover service demands across the extended hours and weekends on a roster basis. Main duties of the job Enthusiastic and forward‑thinking Specialist Occupational Therapist to join a busy, pro‑active, integrated service based across St.

Catherine's Hospital and Arrowe Park Hospital. Responsibilities: complete comprehensive assessments for patients with a wide variety of physical and mental health conditions and complex social issues; implement treatment plans and collaborate with MDTs. Liaise and refer into wider health, social care and voluntary agencies when appropriate to ensure the best possible outcomes for patients, their carers and families.

Requirements: significant post‑grad NHS experience, preferably within a community setting, good understanding of risk management and moving and handling; excellent communication skills; ability to prioritise a large caseload of complex patients; good time‑management skills. The post holder will contribute to service‑improvement initiatives and participate in continuous professional development, supervision and in‑service training. About us Current HCPC registration with the relevant therapy qualification is essential.

Travel is an essential part of this role and therefore access to a car for work purposes is essential. Appointment to the post will be subject to successful interview, references, DBS clearance and Occupational Health checks. Ability to empathise with others. Requirement to have a valid UK driving licence and access to a vehicle for work purposes. Qualifications and Professional Training Occupational Therapy degree or equivalent qualification.

Registered with the Health Professions Council (HPC) as an Occupational Therapist.
