Schedule : Every weekend Days : Friday, Saturday & Sunday Hours : 8:00 AM – 8:00 PM (Full 12-hour shifts) Work Type Remote / Virtual Care What You’ll Do As a Virtual Care Nurse (LPN) , you’ll support high-risk patients and help prevent hospitalizations through proactive, remote care. · Conduct virtual patient visits and clinical assessments · Provide education on chronic disease management · Monitor telehealth data and identify risk trends · Perform triage and early intervention to prevent rehospitalization · Collaborate with interdisciplinary care teams · Participate in care strategy meetings and performance improvement initiatives · Support development of virtual care best practices What You Bring · Active, unencumbered Florida LPN license (required) · 2+ years of home health experience (required) · Strong background in triage & hospitalization prevention · Experience with high-risk or complex patients preferred · Comfortable with telehealth technology & documentation systems · Proficiency in Microsoft Office (preferred) Why Join Concierge Home Care?
