Job Description
Senior Community Nurse Closing date: 13 May 2026 Band 6 practitioners work with a defined caseload of service users and usually as part of a multi‑disciplinary team, providing same‑day/urgent assessment, care planning and care delivery. This will include the provision of highly skilled care, psychological interventions, clinical assessment, risk assessment/risk management and promoting recovery and inclusion. The practitioner operates within a community setting and will be required to work alone but can use team support to manage risks where needed.
The practitioner will be involved in seeing people newly referred to the service, completing assessments for decision‑making purposes and assessing risk for those potentially unknown to services. Job Responsibilities Shift coordination, risk assessments/risk management plans, care planning, triaging referrals, and visits to service users in their own homes. Link with other services, reach service users on wards, prepare MDT notes, attend MDTs and offer psychological interventions as necessary.
Signpost to other services, complete discharge documentation, and complete initial assessments for new referrals. Occasionally engage in project work or contribute ideas/participate in subgroups to provide feedback on specific topics within the scope of professional practice. Provide support to students, where required.
Experience and Skills Demonstrates a passion for working with older people with dementia and other mental health needs. Knowledge Demonstrates an ability to assess, plan, implement and evaluate care and communicate findings using both written and verbal skills, including assessment of risk. Contract & Work Details Salary: £39,959 – £48,117 per annum, pro rata.
