This role supports continuity of care for an adult population within a collaborative, mission-driven environment. The schedule begins at 2 days per week , with the potential to expand based on patient care needs and provider productivity.

Ideal days are Monday and Tuesday, but the center can be flexible. Clinical Details Patient Population: Adults only (18+) Daily Volume: patients per day Clinical Focus: Preventive health services Chronic illness management (HTN, diabetes, COPD, etc.) Wellness and health maintenance Integrated care within a behavioral health setting Responsibilities Provide comprehensive adult outpatient preventive care Manage chronic medical conditions Maintain a daily caseload of patients Collaborate with an interdisciplinary team focused on whole-person wellness Document and coordinate care using Athena EMR Maintain high-quality, patient-centered clinical standards Requirements Active Washington ARNP license (or ability to obtain) Family Medicine background strongly preferred Experience in adult outpatient primary care and chronic disease management Prior work in community-based or integrated care settings preferred Strong communication skills and comfort with electronic health records
