Position Summary: We are seeking an experienced and empathetic Nurse Practitioner (NP) to join our team as a Remote Care Manager. In this role, you will engage with patients virtually to assess, coordinate, and monitor their health care needs—playing a vital part in our mission to enhance quality of life through proactive care management. Key Responsibilities: Conduct remote patient assessments with empathy and professionalism Monitor patients' health status and escalate warning signs appropriately Apply medical terminology knowledge to assess and coordinate care Communicate clearly and compassionately with patients and families Document thoroughly and accurately within multiple computer systems Prioritize daily responsibilities while working independently Provide excellence in patient care and customer service Maintain patient confidentiality and HIPAA compliance at all times Qualifications: Current, active NP license in good standing Minimum 3 years of experience in a clinical or patient-facing setting Minimum 3 years of experience in care management or case coordination Prior experience working remotely preferred Superior verbal communication and active listening skills High proficiency in medical terminology Excellent organizational and documentation skills Professional phone etiquette and strong interpersonal abilities Ability to recognize clinical red flags and escalate care as needed Tech-savvy; able to work within multiple systems efficiently Must have a HIPAA-compliant, private work area at home Why Join Caregenix? Do you have a current NP license in the state of TX, IL, MI, NM, AZ, AL, FL CA, NB or have a compact license? Education: Master’s degree or higher from an accredited school of Nursing and graduate from an accredited Nurse Practitioners program.
