Job Description Job Description Salary: $30.00-$35.00 Trinity Social Services, LLC is seeking an LPN nurse for our Behavioral Health program. This nurse will be an independent care provider, highly skilled in managing patients' mental health and behavioral needs. Primary Responsibility: Promotes and restores clients health by completing the nursing process, collaborating with physicians and multidisciplinary team members, providing physical and psychological support to clients, friends, and families, and supervising assigned team members.
Job Duties and Responsibilities: Providing nursing assessment, AIMS, and interventions to observe, monitor, and care for physical, nutritional, behavioral health, and related psychological issues, problems, or crises manifested in the course of the clients treatment. Maintaining accurate, complete health care records and reports Administering medications to clients and monitoring them for side effects and reactions Recording patient vital signs and medical information Ordering medical diagnostic and clinical tests Monitoring, reporting, and recording symptoms or changes in patient conditions Administering non-intravenous medications Assessing, implementing, planning, or evaluating patient nursing care plans by working with healthcare team members Modifying patient health treatment plans as indicated by patient conditions and responses Clinic schedule 1x-2x monthly Call in medication ordered by NP/PA Meet with clients/guardians to educate on NP/PA instructions, meds, etc., after meds are called in, and to determine questions Conduct a routine follow-up nursing session Requirements: Current GA State licensure as an LPN nurse is required American Heart Association BLS ACLS certification Ability to effectively communicate with clients, families, physicians, and hospital staff Basic computer skills Professional, friendly attitude Experience in mental health or home health care (plus) Experience with Carelogic (plus) Full-time or Part-time
